They claim that if I want to perform rib rhino in their clinics, they will use ready-made autologous rib cartilage tissues for my augmentation. What does it mean ?? Does it mean that the cartilage was harvested from corpses ?? A little bit confused but that doesn't sound bad as I won't be having a scar on my chest after all. How do you all think ?? Bone and cartilage autografts commonly are used. We used rib cartilage graft for dorsal nasal augmentation. The aim of this study was to investigate the long-term outcomes and patient satisfaction of dorsal nasal augmentation with rib cartilage grafts. Thirty-eight patients who had been operated on for dorsal nasal augmentation with rib cartilage graft were included in this study. Operation times, postoperative complications, and revision surgery were investigated by using hospital records. Anthropometric measurements were used for determining the resorption rate of cartilage graft. A questionnaire was used for the evaluation of long-term patient satisfaction. Evaluation of the patients' records showed that average operation time was 116 minutes and there were no complications such aspleural damage. The mean follow-up period was 27.4 months. Nine patients required secondary nasal surgery, including soft tissue augmentation (n = 1), nasal tip revision (n = 3), and reshaping the cartilage graft (n = 5). Long-term anthropometric measurements, comparing preoperative and postoperative values, documented increases in both tip projection (4.2%) and nasal length (1.2%) and an increase in nasolabial angle of 8.2 degrees . Eighty-one percent of the patients who answered the questionnaire rated the nasal size as normal. Nasal symmetry and nasal shape were good in 75% and 66% of the patients, respectively. Results of the rib cartilage grafts that are used for dorsal nasal augmentation are satisfactory for the patients and resorption rates are not high enough to change the shape of the nose. Currently, allografts and autologous tissues are used for nasal augmentation; however, an ideal technique has not yet been described. Although preferred for augmentation of different parts of the body, pure dermal graft use has not been described for nasal augmentation. The authors performed nasal augmentation using a dermal graft in 90 patients in their hospital between 1994 and 2000, and they followed up the patients for 6 months to 8 years. In this article, the early and late results of dermal grafts for nasal augmentation are presented, and their advantages and disadvantages are discussed with a review of the literature. Seems promising. However, some articles say that it can be used to build up to 3-4 mm but if you need more than that, it is hard to achieve with dermal graft. Abstract Autogenous rib cartilage grafts have gained more widespread use in rhinoplasty as dorsal onlay grafts and columellar struts. However, the usefulness of rib as a donor site has been limited by difficulties with postoperative cartilage warping. We hypothesized that the internal stabilization of rib cartilage grafts with Kirschner wires would prevent warping. The costochondral cartilages of a fresh cadaver were harvested and carved into 4 x 10 x 40 nm blocks. A single 0.035-in K-wire was placed longitudinally into the center of each of the study specimens (n = 9), whereas no internal stabilization was utilized in the control group (n = 9). Over a 10-day study period, a mean of 2.2 degrees of warping was observed in the grafts with K-wires as compared to 8.9 degrees in the control group. This indicates that internal stabilization of rib grafts significantly reduces warping (p < 0.001). In a subsequent clinical study, 28 patients underwent placement of internally stabilized columellar struts (n = 19) and/or dorsal nasal grafts (n = 12) using autogenous rib cartilage. At a mean follow-up of 13.5 months (range 3 to 36 months), graft warping was not observed in any patient. Satisfactory aesthetic results were achieved in all but one patient, in whom mild displacement of a dorsal onlay graft occurred. Palatal extrusion of the K-wire occurred in 3 of the first 9 columellar struts. This prompted an alteration in technique with no subsequent extrusions.
