My plastic surgery with Dream Medical Group

I've been a silent reader reading through your posts and plastic surgery journey in purseforum. Thanks so much for sharing your experience :smile::smile:

I've been considering rhinoplasty for a while now and finally have a chance to go to Korea this coming week. Maybe its because of the community around me scaring me with plastic surgery horror stories...but because of this I really decided to play it safe and arrange my upcoming rhino with a clinic where a family member of mine got her rhino done in 2009 with natural and amazing results. Also, I researched it to be really popular with Korean locals and celebrities.

I didn't even consider going to any other clinics for the surgery...because I've had some basic online consultations with them, but no matter how great the medical credentials, friendly, professional or persuasive a doctor may be, I think the most closest comfort you can find, is when you actually know a family member who shares their story with you and is really honest and raw when giving plastic surgery advice. Plus, when i visited Korea back in January I had a good consultation experience with my chosen clinic (about double eyelid surgery). Basically, due to a lot of factors, I found that this clinic would be the best choice for me in getting a "natural" nose. So I booked my surgery a month ago and was all ready to go.

That is until I called the clinic a few days ago.......I wanted to re-confirm that my surgery was actually booked with the specific surgeon I requested (same one who did my family member's nose) before leaving for Korea. But the consultant said that this doctor would only consult with me and create the "design" of my nose while the actual plastic surgery being conducted by another surgeon. This completely enraged me because all this time, I booked in advance for the doctor I wanted and initially got their word that he would be my surgeon. But all of a sudden, he becomes unavailable/busy because he only does breast surgeries now and does not perform rhinoplasties for patients anymore. Their reasoning as to why he cannot perform the surgery is still really fishy to me...I mean they should have told me clearly from the beginning that he would only create the design, but they didn't. I feel as if I have been tricked, and am starting to get really skeptical of doing my surgery with them, despite my family's amazing prior experience with them. They tried to reassure me that he would still be the one creating the design of my nose (which he rarely does for patients since he doesn't conduct rhinos anymore) but would especially do for me.

Anyways, all rambling aside, due to this I really started to consider Dream Clinic after reading about your experience. Dr. Park Yang Soo seems to be a very specialized surgeon. Based on your experience, how do you think I should converse with him in communicating the type of nose I would like? To be honest, unlike many forumers here, the type of nose I would like is very simple, maybe too simple and natural that its not even worth getting rhinoplasty? I'm really not looking to get a sharp defined nose with an amazing nose bridge height.

My nose is the typical asian nose that's round and undefined. I wouldn't say its bulbous but I do have a wide nose base with slightly fat alars which is what bothers me and the reason I decided on rhino. My nose bridge is also pretty low, like a typical Asian. However, my nose is pretty long (starting from between my eyes and downward), not upturned, nothing unusual. To be honest, I think my nose is okay...not overly conscious about it...and am just looking for a minor improvement, that is still reminiscent of my original nose shape.

I would like my nose to become more narrow at the base (from one end of the nostril to the other end) and maybe a slightly higher nose bridge. Really nothing major. Just a slightly more defined nose that will complement my other facial features. I'm planning on getting a 1.5mm - 2mm implant as bridge height is really not a concern for me and using ear cartilage just for the tip of the nose. Maybe an end result close to something like this:

In order to achieve this look, would placing a silicon implant of 2mm be sufficient to make my nose look less wide or fat? I'm really not sure if a silicon implant of only 2mm would be enough to make a nose look less wide, or if it would fix the wideness at all?
Also do you think an alar reduction is necessary? Since my alars are only slightly fat, I'm just really scared that alar reduction would be too much and make me look like reminiscent of this:

Which would be a complete nightmare........

uhhhh design your nose? What does that even mean? lol....Someone can't just come in and draw out a design for your nose then send it to the surgeon who then makes your nose. It doesn't work like that lol. My assumption is that the surgeon who was supposedly "designing" your nose no longer wants to perform rhinoplasty because he is simply not that good at it and doing so will risk affecting his reputation. Notice how the mid range to larger clinics have different surgeons assigned to different procedures? That's because surgeons will tend to avoid performing procedures which they do not specialize in as their reputation is their lifeblood.

I think the best way for you to communicate to any surgeon, the kind of nose you want is to simply photoshop yourself. It can't get more accurate than that. I actually cannot open the links you posted so I have no clue what type of nose you are after. However, if you are after extremely minor change it might be best not to have any surgery as i've found individuals with such aspirations tend to have revision after revision after revision trying to look more like they used to. Its like a lose lose situation.

Also there was a forum member who did her nose with dr Park in 2013, who used a 2.5mm implant and it produced a very natural looking nose. Was a subtle change but noticable improvement. I'll pm you her forum id once you can private message and you can speak to her about her experience. I don't want to post it here cos there's a number of rude ppl who will just randomly pm her and ask for things right off the bat.

First of all, hope all is well with you.
Can I ask you why Dr. Kim did your acculift? Was it because Dr. Park didn't specialize in Acculift? But I've heard from one forumer who did Acculift with Dr. Park @Dream, which seems to me that Dr. Park can perform that procedure.

i don't think there's such a thing as specialize in acculift. That procedure is so minimally invasive its like a botox with sedation. Mine literally took 10 mins only to perform. In contrast, procedures like facial bone contouring and rhinoplasty have specializations. Dr Kim did my acculift because being the facial bone contouring specialist, I initially consulted with him about getting a cheekbone reduction but after viewing my 3d ct he recommended the acculift.

thank you for your help so far and also for the list I learn so much from you already! i don't know anybody else who got so much medical knowledge and teach me so much. I read that you say your nose got red colour on the bridge part? Is it still there? I also got that problem you know because of my rib nose surgery. I thought that after remove the rib will be better but the red part still there. If I get another nose surgery I scared the red part will still be there. Do you know what i can do to make the red color disappear?

Hey Moreen. For you to be experiencing redness after you removed your rib cartilage is a bit odd as I recall you mentioning you took it out more than a year ago? For such a situation the main factor to look out for is rate of fluctuation. That is, should redness be getting worse as time goes by it may be a low grade infection or chronic inflammation. However, when the density of redness fluctuates on and off, its becomes a form of pigmentation.

My nose for example has donor thigh tissue in it. The reason it is still red and fluctuating is because i had an extrusion so the graft is growing into my damaged skin. The second reason is that the fascia tends to increase blood circulation on and off. That is the reason behind my redness as mine was a reconstructive rhinoplasty.

I suspect, if yours is fluctuating too it would be because by removing the rib cartilage from your nose, the surgeon is yet again seperating the skin and soft tissues of your nose. Doing so always does some damage to the nerves and vessels in the area which aid in fluid drainage. As this process occurs more often, the healing time for pigmentation increases. It doesn't happen to everyone since its quite rare but its by no means a cause of concern. Unless of course it just gets redder and pussier and tender and then crusts up. Then you should be concerned as that is signs of an infection.

Dr Park actually recommended me to try a laser to help with the redness. Its called vascular laser treatment for dilated blood vessels. Im actually undergoing that treatment in oct so i can let you know how effective it is :smile:

Ok so because yours got fascia inside so it become red? Maybe mine also got fascia inside because before i take out rib my nose already was red. Doctor thought was infection so keep giving antibiotics but redness never go away. Even after I take out rib still same problem. Do you think they use fascia in my nose also and then leave it inside but take out the rib?

Yes please let me know about the vascular laser treatment and tell me if good or not and where can i find that laser?

no it is unlikely they even used fascia to begin with because from my understanding the fascia is not often used for rib graft unless it is diced rib. Diced method requires the fascia to hold the pieces together, but whole rib does not. When fascia is used on non diced rib, the logic is that it minimizes risk of warping. However that is as of now a debated technique so its not done often.

Um actually i don't know where offers the vascular laser LOL. I looked at dream's dermatology menu and its not on there. Initially i had just assumed they have it because Dr Park recommended it. Maybe they do but its not part of the dermatology department because some of their lasers like the one they used for evening up my fat graft are in another department. Ill let you know when i find out.

I've read that a lot of ppl have had their noses done at Dream but I haven't really read much about eyes yet. But many suggest not to do both at the same time because it'll be a much rockier recover versus doing one procedure at a time. Hope that helps!

I know this is not in the same category of surgeries you did but would you (or anyone) really know the price they quote for mandible jawline reduction?

P.s I've been a long time lurker and just also wanted to thank you for your constant and helpful contribution to my PS research. You've been a great help to this community and even if you cant answer my question (I'm just awaiting quotes anyway) - you've helped me so much more than you would have ever known!

If you are asking specifically for dream, i don't know as I've never inquired about those procedures.

In general tho, jaw reductions can range from 5-8 million depending on the clinic you go to. And V line can easily cost from 7-12 million. The most expensive procedures are the bone manipulation surgeries and the face lifts after all.

The price you get quoted depends on the requirements for your specific case and also the specialty of the surgeon. I believe a reputable surgeon would not charge less than 6.5 for the jaw reduction, assuming you are only getting one procedure with little room to bargain that is. I mean if you do some research you will find that for example, a clinic like VIP which attempts to brand itself as specializing in rhinoplasty, can charge as high as 8-10 million for primary rhino. In contrast their other procedures are significantly cheaper. Same with Teium. Their branding strategy focuses on the eyes. Hence their eye procedures cost more than most clinics but their other procedures are cheaper. Then you also have to consider the clinic range. In terms of pricing, OZis at the low range. JW is at the lower - mid range. Dream is at the Mid-Higher end depending on the surgeon involved and procedure and April 31 is at the Higher end.

I think I'm just going to go ahead, book the consultation and not bother too much with the price for now and decide then. I just hope Daphne (who you had) is working there still as the lady I've been corresponding with via email is not really answering my question.

I'm actually quite annoyed now with emailing back and forth and not getting a straight answer. I asked plainly what is the ballpark for jawline reduction and got given a bunch of questions. Sorry to be impatient but why do they need to know my DOB and personal details if I'm just asking pre-consultation questions about pricing. Anyway, they replied back with answers that tailor towards the chin but I mentioned specifically only mandible jawline reduction.

I'm wondering whether I am missing something here...
