teeteetee Posted April 28, 2017 Posted April 28, 2017 Hi everyone, I may consider revision rhinoplasty a few years down the road. I think the current (silicone) bridge height is alright. My area of concern is refining the tip and fixing mouth protrusion. I think the answer that I found was septal extension graft where the tip is extended forward. This lessens the appearance of mouth protrusion because the area where the nose and upper lip meet is pulled forward. My question is, is it possible to achieve all of this through closed rhino if I wanted to just refine the tip and not touch the bridge?
k-couture Posted January 25, 2018 Posted January 25, 2018 there's a several threads about this. ill break it down: closed rhino is suitable under the following conditions: 1) you don't have a bulbous tip 2) you don't require extensive seg Its limited in what it can acheve with bulbous excision and seg pretty much. also nose surgery doesn't fix mouth protrusion. Usually that is die to prominent upper lips and/or over bite, which requires other procedures to fix. A more defined nose can slightly, SLIGHTLY balance it out a bit more by creating an illusion. But seg when overdone, or made too long can actually worsen it.
hohohalmoni Posted January 25, 2018 Posted January 25, 2018 From my point of view, the only adventage of a closed rhinoplasty is that it leaves no outer scar. Except that, open rhinoplasty is way beneficial since it provides more view and allows the surgeon to freely navigate and do his job more accurately. Especially if it's a revision, and if it's the tip that you mainly want to improve, accuracy is even more demanded (bridge augmentation is simple). Also, since closed rhinoplasty only uses one side of the nostrils to approach the whole nose, in some cases, assymetry occurs after surgery. Lastly, you should not expect rhinoplasty to improve your mouth protrusion. Though it can be slightly less noticeable after rhinoplasty, a part can never be fixed by correcting another part.
