Hey all, planning to go to Seoul for ptosis + vline+ zygoma reduction + rhinoplasty.

I notice that while zygoma reduction reduces the width of the face, making it look nice from frontal view, it does nothing for the profile view which still has Asian pancake face. Is there a way to shave the bone and push it forward/downwards to give lower cheekbones? Basically my jaw is super square and juts out more than the upper half of my face which is flat in comparison and you can see the other eye (the nose is not a barrier)

I'm looking for something more permanent than fillers and less invasive than bone implants (asking for the impossible I know!) to get lower cheekbones.

So basically my current face looks something exactly like this:

As you can see, cheekbones are wide and there's an overall flat look to the face. I'm looking to achieve a slimmer, more streamlined look with forward projection and bone structure similar to the girl below:

HOWW???

What surgery can I get to achieve this look? Purely bone structure wise.

Hello! I have some what similar problem as you. I have protruding mouth. I think what we need is ASO. Anterior Segmental Osteotomy. Im not sure what clinics do this procedures though.

Just googled what you mentioned and wow, its like a whole facial transformation going on and it looks exactly like something I need. Is it also filed under the term zygoma reduction or is there another word for it?

I was reading up on zygoma reductions in this forum and basically everyone complained about facial sagging after a year or so. I'm having second thoughts about getting that combined with jaw surgery. I also suspect my skull is naturally large and it's not just my cheekbones that are wide. :sad:

Can I get away with just zygoma reduction or do I also need to do square jaw reduction? I was told by a surgeon that my jaw isn't that big and it's mostly masseter muscle. I've tried botox and while there's some improvement, my jaw is still large as seen in the photos below. (Ignore double chin fat)

Also, not sure if zygomas protrude more in a 45 degree angle or the front or size. And one side is bigger and founder than the other. This whole thing is disturbing my mental peace. Do you think I need zygoma reduction? Is it worth the sagging skin?

You can do Botox to reduce the size of your muscles or have surgery to shave down or remove some masseter muscle. Although for masseter muscle removal make sure you find an experienced surgeon or else if it heals wrong you won’t look right.

My cheekbones are also protruding to the sides. When I was younger, I used to have plump cheeks like the last photo that you portrayed as the "front view".
With age, I gradually lost the fats in that area.
Maybe you would want to look into fat grafts first instead of something so drastic like a cheekbone/zygoma reduction. Most forumners have face sagging 1 year later after cheekbone/zygoma reduction.

That's the thing, it's not fat, it's actual bone sticking out - but yeah, if you only saw my face without seeing my body (I'm only 40kg) you'd be forgiven for thinking I'm overweight. I know reduction causes sagging but I guess it's better than looking obese with harsh angles? IDK. I've made appointments with TFD and zeah dental so we'll see.

If you're wondering whether you should get a jaw bone reduction, my opinion is you should get the muscle reduced before any other surgery to decide whether you want to proceed with jaw bone and zygoma together. If you get only the zygoma and muscle reduction and it turns out muscle wasn't enough, you'll end up with a square face. I'm not necessarily encouraging adding procedures, but just be aware that insufficient jaw reduction could result in a square face if you do zygoma.
