i didnt even have enuf septal to make a tip strut. so unless youre an asian cyrano i think it doubtful you will have enuf either. i can understand septal for caucasians as they have lots of septal and already existing bridges. but for asians i never heard of an instance of septal for bridges. sometimes ear and rib, not septal. and you really dont want to screw around with a collapsed septum. flame was supposedly having her bridge done by shah around now perhaps she will report back and let us know how it went. and there was one other as well.

since he will probably use perichondium to line the dorsum he will have another surgical site anyways.

Hi all,

It's Day 3 post-op for me. Had a revision to take out my silicone implant and replaced with rib cartilage by Dr. Shah in Chicago. Too tired to make a lengthy post about my surgery so please pm me if you want details.

Just wanted to make a few general comments...

for those who feel strapped by time and feel you "need" to make your decision...please make sure this is not in haste. It is better to wait a year if your summer window is closing. You DON'T want to have to go for a revision later down the road...esp if you pick an implant that is harder to take out or will pose future problems. My revision was an open procedure and I have to say... it's a real bastard of a recovery. This will definitely be my one and only revision. Don't want to go through this again.

A note on Dr. Shah: if you are close and can go consult with him... you should. He is as nice and caring as surgeons go. Very patient and listened to all my concerns. And my revision was quite complicated but he managed to pull it through. It's too early for me to tell if we achieved my goals because of all the swelling... but my rib donor site doesn't hurt at all and I hardly have any bruising on my face. I can tell he was very careful and gentle. HOWEVER, picking a surgeon is a very personal choice and has everything to do with what your nose is currently. Depending on whether this is a primary or revision and what your natural structure is... he may or may not be right for you. But you need to consult with the docs (in person if possible) and express your case and work out all the details and pick the one you feel can give you the results you want. We all want different things so naturally our choice of doctors should vary.

A note on L-shaped silicone: DON'T EVEN CONSIDER THIS OPTION! This is what I had for 15 years and I was one of a few that didn't have problems within this time frame. But it had started to develop a bend and was on it's way to extruding. Also, the skin on my nose was "shrink-wrapping" around it and it is very obviously fake looking. Silicone implants can photograph well (even mine did at certain angles) but in person, I can always tell when someone has it in their nose...

As for the continuiing debate of what is best as augmentation material... I don't think this is a black or white issue. REally depends on what you want accomplished. The decision is based on so many factors :if you don't mind future revisions down the road, if you want to make it easier to change later on, if you are more scared of extrusions or infections than warping issues, etc. Sorry, no perfect answer. However, I have researched this topic quite a bit and if you send me a pm, I can try to give you my 2 cents on your specific nose or situation. I can't respond to this question with a general commentary because I think the actual answer is actually very specific to the person asking. For example, for me PERSONALLY, I felt it was best to go with rib because I had a bad experience with silicone and was turned off synthetics. Also, I needed much height so using ear or septal cartilage were not options for me.

Make damn sure u want to do this... I'm 9 months p.o. and my nose looks great, but i sometimes have panic attacks thinking about getting another surgery..

i got an I-sillicon implant with cartilage grafting. no one noticed ne thing. which kind of makes me feel like it was such a waste.

sometimes i dun mind getting my nose, but i have this awful feeling right now that i got my nose done. U can only really tell from the side that i got something done. And nose jobs, if u really think gabout, doesn't really do **** for you. it doesn't make u look better or net hing unless u have a really low bridge with a flat and bulbous tip.

although i got mine in korea, i was wondering if ne one ever had complications, and at what time period they did. any response would be appreciated, considering I'm freaking out at the moment

i dont get it. you are freaking out because no one can tell you had a nose job so you want to get another one so they can tell???

the profile makes an enormous difference in how attractive others perceive you to be. dont underestimate it just cause you cant see it for yourself. its rare people see you directly head on and they judge your looks first from the side. if your profile doesnt look good it makes a poor first impression. if your profile looks good then its mission accomplished.

but people close to you really dont pay much attention to that kind of detail unless they are artists for a living. so its not surprising people dont notice. i had bridge and tip and chin and none of my relatives noticed. they noticed my eyes and thot i had lip surgery or something. but bottom line i look better than before. the nose and chin is all about subtle changes and it all adds up.

if you want something drastic, get your eyes done if you havent already. theres no missing that and it makes a big difference. i havent seen your nose but it sounds like its as it aught to be.

Call me crazy (also, I'm not asian so perhaps I have a different opinion on all this) but I really loike all the before pictures better. I think this is a perfectly beautiful nose to begin with.. must be missing something.

I'm getting lots of questions about my surgical experience with Dr. Shah: so a general note here and then I will answer more specific ques via pm:
-Dr. Shah has been absolutely fantastic... I never felt rushed during the consult (which ran overtime because I was so picky about my morphs) and he was extremely patient with all my questions (he is as nice in person as he appears via email). That was reassuring to me when I met him because sometimes docs can seem nice via email but then short and curt when you meet them
-listened to all my concerns and remembered everything I wanted from my copious previous emails to him
-post op care was excellent...he dropped by my hotel room to visit and phoned over the weekend to make sure I was alright
-surgical center staff was excellent... caters to your needs ... just have to specify what your special needs are and they will take care of you

About using rib in asians: I think most docs who offer the variety of augmentation material (silicone, goretex or autologous) do so because :
1)achieves broader range of patients
2)want to give their patients more personal choice

I think surgeons who specialise in autologous materials have a niche in the rhinoplasty business because it is a far more complex surgery so their training and experience need to be more unique. As far as I'm concerned, autologous (esp using rib cartilage) surgeries are more complicated and require more technical expertise.

As far why or why not use rib in asians, I think the answer lays with us... most asians I know don't want invasive surgery or additional surgical sites. They are happy to have a quick and easy surgical insertion of silicone (some docs can do this efficiently with endonasasl techniques) but then the majority of asians I know that have had silicone implants ended up getting revisions later down the road due to extrusion, movement, infection or implant visibility.

I think rib cartilage is a suitable implant material for any person who needs the height not achievable by septum cartilage, regardless of ethnicity.

And to everyone that asked me whether I regret getting my silicone implant:yes, I do and I'm sorry but I would never recommend it to anyone based on my own personal experience.

What time frame in September do you have in mind? Are you having a rhinoplasty surgery too? Which doctor that you have selected?

Sorry Sandy for the delay. The doctors in Korea suggested that I stay in Korea at least 2 weeks. I probably stay 2 and 1/2 weeks just in case.

while i agree with you on the 2nd ones, i think the 1st is an improvement. her nose had a snarl look to it and wide before and after its better.

photos have difficulty translating 3 dimensional frontal views. in asians the lack of bridge height and monolid often makes the upper face look tired/washed out. so in real life i think you would notice a more balanced face with the added bridge height. profiles will definitely look better as well.

Just had appointment with my doc... and he assured me that ear cartilage do raise the bridge ... ... Im still very confused, from what i read ... the general impression is that only rib cartilage can do that. Help?? I did show him pic of a eurasian model's nose ... and he actually assured me that i can reach that height...

Can the ear cartilage be taken out without causing any damage (tissue damage, scarring) if revision surgery is needed in the future?

Hi thph.. i am also planning to get my nose done in sep.. was tossing up between taiwan or korea.. currently corresponding with doctors from both countries so if you are going, it would be great to have some company.. :smile:
